Job Summary of RN Case Management:
Vetted is seeking an RN - Case Management for a travel assignment. This role involves managing patient care and coordinating services, with 8-hour shifts, 5 days a week, for a 13-week duration.
Job Duties and Responsibilities of RN Case Management:
 Coordinate patient care across multiple settings and services.
 Develop and implement care plans tailored to individual patient needs.
 Collaborate with healthcare professionals to ensure comprehensive patient care.
 Monitor patient progress and adjust care plans as necessary.
 Provide patient and family education regarding care plans and resources.
Travel Requirements of RN Case Management:
 Must live at least 60 miles from the facility to qualify for travel rates.
Qualifications and Experience of RN Case Management:
 2 years of experience in case management or related field.
 BLS certification.
 National and state RN certifications as required.
