Hybrid CNA/MA Remote Full T ime

Job Summary We are seeking a detail-oriented and compassionate Certified Nursing Assistant (CNA) or Medical Assistant (MA) to join our team, supporting patients and healthcare providers through both on-site vitals collection and remote patient monitoring/chronic care management. The ideal candidate will visit assigned facilities daily to obtain patient vital signs, then transition to remote work from home to review data, document information accurately, and provide timely communication to care teams. This role ensures patients receive the highest standard of care through both in-person and remote support. Responsibilities • Travel daily to assigned facilities to obtain patient vital signs. • Review and monitor patient health data through remote platforms after in-facility rounds. • Document patient information accurately and promptly in electronic health record (EHR) systems. • Communicate important updates or concerns to nursing staff and healthcare providers. • Assist in patient outreach for wellness check-ins, reminders, and care coordination. • Participate in quality improvement initiatives to enhance patient outcomes. • Maintain confidentiality of patient information and uphold HIPAA regulations. Requirements • Certified Nursing Assistant (CNA) or Registered Medical Assistant (RMA) certification required. • 1–2 years of healthcare experience, preferably in remote patient monitoring, chronic care management, or a similar role. • Strong communication and interpersonal skills. • Ability to work independently and manage multiple priorities in a fast-paced environment. • Proficiency in electronic medical records and comfortable using healthcare technology. • Bilingual in Spanish preferred.
