Hospice Physician - Remote

Job Summary: The Hospice Physician – Remote will function under the supervision of the designated hospice Medical Director. The hospice physician as an employee or contracted employee of the hospice provides core patient services via virtual connection under the supervision of the medical director and in conjunction with the patient’s attending physician. The hospice physician assists with the palliation and management of the patient as related to the patient terminal and related illness.

Essential Functions:
• Participates in and promotes core care delivery process related to the patients in the hospice.
• Supports an interdisciplinary team meeting via virtual platform when delegated by the Medical Director.
• Proactively keeps abreast of hospice and palliative care trends, best practices and any anticipated compliance changes.
• May act as a liaison for partner organizations.
• Provides telephonic medical support and remote clinical guidance to patients, caregivers, and interdisciplinary staff as needed; assesses patient needs, provides medical guidance, and supports clinical decision-making as part of regular duties.
• Practices within the accepted standards of the American Medical Association and certified by an American Board of Medical Specialties (ABMS)-recognized Board, or the Osteopathic equivalent.
• Provides support to medical staff to perform field-based virtual joint visits.
• Proactively assesses, leads, participates, and promotes a culture of clinical excellence and innovation at the employee, departmental and organizational levels aimed and continually improving employee clinical performance and engagement.
• Participates via virtual platform in meetings at the individual, group and organizational levels, both within and external to the organization, with ease and comfort to achieve desired outcomes.
• Provides educational support to the orientation process, referral sources, facilities, and community.
• Proactively works to coach and/or mentor clinical staff and management to improve their clinical performance.
• Ensures internal and external education, training and activities for self and staff to promote personal and professional growth and to ensure staff competency is maintained at all times.
• Proactively promotes census growth within designated region(s) and seeks to offer care that meets the unmet diverse needs of the community.
• Proactively participates, promotes, and collaborates externally, with various individuals and/or groups, to establish various benchmarks and standards as it pertains to hospice and palliative care, locally, statewide, and nationally, when appropriate.
• Upholds the organizational Code of Conduct, policies, procedures, protocols and processes and all regulatory and legal requirements.
• Models the NorthStar standards to care for every person, every time, 100% of the time.

Supervision:

The Hospice Physician will clinically collaborate with the designated Medical Director for virtual patient care guidance related to hospice and/or palliative care. The Hospice Physician will report to the Medical Director for management and professional guidance.

Qualifications:
• Licensure as a Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) in the State of Michigan.
• Valid Drug Enforcement Administration (DEA) license for controlled substances.
• Recognized performance in maintaining highest standards with regard to good medical practice, ethical behavior and palliative care, including scientific and clinical excellence, demonstrating psychosocial and spiritual knowledge.
• Must possess excellent interpersonal skills, sound judgment, effective organizational, prioritization and follow-through skills, attention to detail, tact, dependability, emotional intelligence, the ability to maintain confidentiality and promote positive, constructive relationships with communication and collaboration at all levels.
• Capacity to access patients regardless of location.
• Must be able to read, write and speak English fluently and be able to communicate orally and in writing in internal and external relationships for all essential job functions.
• The physical demands of the position include: vision, effective speech and hearing for extensive telephone contact; repetitive motion; traveling; driving or riding in motor vehicle; standing, sitting, walking, bending, reaching, and stretching; lifting up to forty five (45) pounds unassisted and the ability to assist in lifting patients using appropriate lifting techniques and/or devices.
• Must have reliable internet connectivity to join IDT, team meetings, and virtual patient visits. Must possess adequate computer skills to maintain EMR proficiency, electronic prescribing, and electronic document processing
• Must be eligible to work in the United States.
