The Hospice Community Liaison Officer at Legacy Now Hospice is responsible for developing and maintaining relationships with referral sources to enhance the hospice patient census. Responsibilities
Β Develop and sustain relationships with referral sources.

Β Build a consistent referral pipeline to increase hospice patient census.

Β Create and implement a targeted marketing plan focused on hospice patients.

Β Meet with hospitals and families to introduce and facilitate admissions into hospice services.

Β Ensure patients receive the highest standard of care.
Requirements
Β Active RN License in Texas (preferred but not necessary).

Β Valid Driver's License and Auto Insurance.

Β Minimum of two years in hospice nursing care for adults.

Β Proficiency in Microsoft Office Suite and Google Docs.

Β Demonstrated success in a hospice healthcare marketing role.

Β Experience in hospice care or family support, either professionally or through volunteering.
