We are seeking a part time Scheduler/Intake Specialist for Friday, Saturday, and Sunday hours. The Scheduler/Intake Specialist serves as a vital first point of contact for patients, families, and referral partners, supporting the timely and accurate coordination of home health and hospice services. This role is responsible for receiving, processing, and managing referrals to ensure patients are admitted efficiently, compliantly, and with a high level of professionalism and compassion. The ideal candidate is detail-oriented, organized, and comfortable working in a fast-paced healthcare environment. This individual must be able to manage multiple priorities, work independently, communicate effectively with clinical teams and referral sources, and ensure all intake processes align with regulatory and organizational standards. The Weekend and Evening Scheduler & Intake Specialist is responsible for supporting home health and hospice operations during evenings and weekends. This fully remote role ensures uninterrupted patient care through timely referral processing, clinician scheduling, and after-hours coordination. Key Objectives Ensure timely, accurate processing of home health and hospice referrals during after-hours periods Maintain clinician coverage for scheduled, urgent, and PRN visits Support safe, compliant admissions and continuity of care Provide consistent, professional, remote support during evenings and weekends Assign visits based on clinician licensure, scope, geographic coverage, and availability Respond to call-outs, coverage changes, and urgent visit needs in real time Enter visit changes, administrative blocks, and on-call assignments accurately Communicate schedule updates clearly with field staff and leadership Requirements High school diploma or equivalent required Coursework or experience in healthcare administration, scheduling, or intake preferred 2–4 years of experience in home health or hospice scheduling, intake, or healthcare operations Experience working evenings, weekends, or after-hours coverage strongly preferred Demonstrated success working remotely and independently Strong organizational, time-management, and critical-thinking skills Excellent verbal and written communication skills Strong attention to detail and data accuracy Proficiency with MatrixCare (preferred) and standard office technology (email, phone, remote systems) Reliable internet connection and private remote workspace required Work Environment & Schedule Expectations Fully remote position Requires scheduled availability during evenings, weekends, and some holidays Fast-paced after-hours environment with real-time decision-making Apply View All Jobs Powered by Payroll & HR Software
