Educate patients and families on wound care management and health maintenance. Collaborate with healthcare teams to ensure comprehensive patient care. Document patient care activities accurately and timely in electronic health records. Provide compassionate support and emotional care to patients and their families. Stay updated on best practices and advancements in wound care management. Participate in ongoing training and professional development opportunities. Requirements: Current LVN license in California with Wound Care Certification. Minimum of 1-2 years of experience in home health or wound care settings. Strong assessment and critical thinking skills. Excellent communication and interpersonal abilities. Compassionate demeanor with a patient-centered focus. Ability to work independently and as part of a team. Proficient in electronic health record systems. Current CPR certification and knowledge of infection control practices.
