Prohealth Home Care is seeking an empathetic Home Health Licensed Vocational Nurse (LVN) who will aim to make a positive difference in our community. Home Health Licensed Vocational Nurse Job Responsibilities:
• Provide direct patient care under the supervision of a registered nurse
• Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes
• Implements plan of care initiated by the registered nurse
• Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (Home Health Aides, Occupational Therapists, Physical Therapists, Medical Social Workers, Speech Therapists, Dietician) to the registered nurse and or Clinical Supervisor/Nursing Supervisor

Home Health Licensed Vocational Nurse Qualifications:
• Valid LVN License to practice in CA
• Current BLS/CPR certification
• Valid CA Driver's license
• Minimum one year Clinical experience as LVN/Home Health is preferred
