Role Description
The Wellness and Recovery Specialist - Certified Peer Specialist is responsible for care coordination and/or care management activities focused on the wellness and recovery of members.
β’ Responsible for coordinating and managing care activities with a focus on the wellness and recovery of members.
β’ Identify opportunities to engage members and their families in forming a supportive recovery network.
β’ Develop and implement onsite psychiatric discharge planning education at Recovery and Resiliency sites.
β’ Collaborate with Stabilization Teams, advocating for members in discharge planning education, resolving barriers, and facilitating service transitions.
β’ Serve as a resource for staff with decision-making and problem-solving.
β’ Initiate and maintain contact with assigned individuals and providers to assess members' responses to services.
Qualifications
β’ Requires an AA/AS and a minimum of 2 years of experience in health services or behavioral health, or a peer support services role, or a combination of education and experience providing an equivalent background.
β’ Valid, active, certified peer specialist.
Requirements
β’ Knowledge of care coordination and case management concepts is highly preferred.
β’ A BA/BS is preferred.
