Main Responsibilities:
Care Coordination: Provide comprehensive care coordination services to adults and children in the SMI/SUD population, ensuring timely access to medical, behavioral health, and social services.
Patient Engagement: Foster strong relationships with patients and their families, providing education, support, and advocacy to promote self-management and empowerment.
Community Outreach: Develop and maintain partnerships with community-based organizations, healthcare providers, and other stakeholders to expand access to care and resources.
Documentation and Reporting: Maintain accurate and detailed records of patient interactions, care plans, and outcomes, using data to inform quality improvement initiatives.
