Health Outreach Specialist-Bilingual Spanish/English (Remote)

About the position

Responsibilities
• Provide navigation assistance and health education to members and providers.
• Conduct outreach related to Population Health prevention and wellness initiatives.
• Answer calls and provide outreach calls as part of the PartnersACCESS Call Center team.
• Maintain data of outreach events, activities, and contacts.
• Coordinate and conduct health initiative events in the community.
• Ensure HIPAA confidentiality compliance in all scope of work.
• Plan and execute health outreach and education engagement.
• Collaborate with Population Health, Care Management, and other departments to address updates to processes and SOPs.
• Attend meetings pertaining to health topics such as prenatal care and diabetes management.
• Participate in agency performance improvement initiatives and quality assurance activities.
• Assist with EPSDT wellness visit reminders and management per NC Medicaid contract and NCQA standards.

Requirements
• Bilingual in Spanish and English is required.
• Associate's degree or higher in healthcare or related health field is required.
• Minimum of 2 years' experience working with public healthcare and related services with families, special populations, and community stakeholders, or equivalent lived experience.
• Thorough understanding of healthcare, managed care, health disparities, and chronic medical conditions.
• Experience in health coaching and outreach is preferred.
• Exceptional interpersonal skills and effective communication ability.
• Strong time and project management skills.
• Proficiency with Microsoft Word, Excel, and PowerPoint.

Nice-to-haves
• Registered Nurse or Licensed Practical Nurse licensure is preferred.
• Bachelor's degree with at least two years of progressive experience in public health, mental health, or substance use disorder is preferred.
