• We are urgently hiring a Nurse Practitioner for a full-time opportunity in a community-based healthcare setting.• The provider will work under the supervision of a physician while delivering comprehensive primary care to a diverse patient population.• This role offers a collaborative, mission-driven environment focused on high-quality patient care and community health services.Job Responsibilities: • Perform accurate patient assessments, diagnoses, and treatments
• Conduct physical examinations, review medical histories, and monitor vital signs
• Prescribe and dispense medications in accordance with regulations
• Develop and implement treatment plans with patients and care team members
• Provide preventive care and behavioral health support
• Educate patients about disease management, medications, and preventive care
• Refer patients to subspecialists when appropriate
• Collaborate with multidisciplinary care teams including behavioral health and wellness providers
• Maintain accurate and timely clinical documentation
• Participate in team meetings and quality improvement initiatives
• Provide coverage for other clinicians when neededQualifications: • Master’s degree or higher in Nursing
• Licensed Nurse Practitioner in California
• Current DEA registration
• Minimum 2 years of clinical experience
• Background in Family Medicine, Internal Medicine, or PediatricsWorking Hours: • Full-Time Schedule
• On-call rotation requiredSkills: • Strong patient assessment and diagnostic skills
• Primary care and preventive medicine expertise
• Patient education and disease management skills
• Multidisciplinary team collaboration
• Excellent communication and interpersonal abilities
• EMR documentation and clinical record management
• Cultural competency and community-based care approach
