Responsibilities
Ā Assess, plan, and implement individualized care strategies for members.

Ā Identify and initiate referrals for member transitions to community services and assist with social service programs.

Ā Manage the care plan throughout the continuum of care as a single point of contact.

Ā Communicate with stakeholders to ensure quality coordinated care and services are provided promptly.

Ā Advocate for members and families to ensure their needs and choices are represented and supported by the healthcare team.

Ā Visit Medicaid members in their homes and other settings, including community centers and hospitals.
Requirements
Ā Current, unrestricted RN license in the state of Texas.

Ā 2+ years of experience working within the community health setting or in a healthcare role.
,
Ā Intermediate level of computer proficiency, including Microsoft Outlook and Teams.
,
Ā Reliable transportation and ability to travel within the assigned region.
,
Ā Access to a designated quiet workspace at home with the ability to secure Protected Health Information (PHI).
,
Ā Reside in a location with access to a UnitedHealth Group approved high-speed internet connection.
Nice-to-haves
Ā 1+ years of experience with long-term care services and support, Medicaid or Medicare.
