Advocate for patients and families to ensure their needs and choices are represented and supported.
Requirements
Active, unrestricted LVN/LPN license for the state of Texas.

2+ years of clinical experience in a health care role.
,
Intermediate Microsoft Office experience, including Word, Excel, and Outlook.
,
High-speed internet access in residence.
,
Reliable transportation and ability to travel within the assigned region.
Nice-to-haves
1+ years of experience in a community health setting.

Experience with electronic charting.
,
Experience arranging community resources.
,
Field-based work experience.

Home health experience.
