As a Field Community Health Worker, you'll play a vital role in managing complex medical or behavioral needs for our members, working collaboratively with our interdisciplinary clinical care management team to advance interventions and promote health equity.
Key Responsibilities:

Partner with clinical care management team to manage complex members in the community
Locate and engage members in the community, developing individualized plans of care that focus on the member's definition of success
Conduct screenings to assess member medical, behavioral, functional, cognitive, and psychosocial needs
Research, locate, and make referrals to community resources to address member-specific care or socioeconomic needs
Perform targeted activities and provide education to support HEDIS/STAR gap closure and health promotion

Requirements:

High School Diploma/GED (or higher)
3+ years of experience working within the Community Health setting in a field-based healthcare role
2+ years of experience working within the local community, with demonstrated knowledge of culture and values within the community
Intermediate knowledge of Medicare and Medicaid benefits

Working Schedule and Location:
Schedule is Monday through Friday between 8 am and 5 pm, with a home-based office and local travel up to 25% (mileage reimbursed at the current government rate). Locations will vary within the following counties of Florida: Hardee, Highlands, Hillsborough, Manatee, and Polk.
