Field-Based HSS Clin Coordinator - Barton, Ford, & Finney counties and surrounding areas
About the position
The Field-Based HSS Clin Coordinator at UnitedHealthcare is responsible for providing person-centered care coordination for individuals receiving Long-Term Services and Supports (LTSS). This role involves assessing, planning, and implementing individualized care strategies, managing service/support plans, and advocating for clients' needs. The position requires travel within a designated region and offers the flexibility of remote work for local residents.
Responsibilities
ā¢ Assess, plan, and implement individualized care strategies for clients.
ā¢ Identify and initiate referrals for social service programs.
ā¢ Manage the person-centered service/support plan throughout the continuum of care.
ā¢ Communicate health-related information to stakeholders to ensure quality care.
ā¢ Advocate for clients and families to represent their needs and choices.
ā¢ Conduct home visits and in-person visits to various care settings.
ā¢ Serve as a resource for community care coordinators.
Requirements
ā¢ Bachelor's Degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or related human services area.
ā¢ 2+ years of experience in a community health setting.
ā¢ 1+ years of experience with long-term care needs.
ā¢ 1+ years of experience with home and community-based waivers in KS.
ā¢ 1+ years of experience using MS Word, Excel, and Outlook.
Nice-to-haves
ā¢ Licensed Social Worker or clinical degree.
ā¢ Background in managing populations with complex medical or behavioral needs.
ā¢ Experience with electronic charting.
ā¢ Experience arranging community resources.
