Job Overview We're seeking an experienced and compassionate Staff Nurse to provide mental health assessments and treatment to patients with psychiatric symptoms, cognitive deficits, and/or behavioral problems. As a key member of our team, you'll work under moderate supervision to deliver high-quality patient care, support VNS Health staff and caregivers, and contribute to the development of treatment plans that make a meaningful impact. Key Responsibilities Conduct comprehensive psychosocial assessments and develop treatment recommendations for patients with complex mental health needs. Travel to patients' homes and/or other facilities to deliver direct care, utilizing approved transportation options and navigating varying environments. Provide nursing care in accordance with Agency policies, practices, procedures, and Standards of Nursing Practice, ensuring a high level of patient-centered care. Offer a range of services, including advocacy, psycho-education, cognitive/behavioral interventions, monitoring of mental status, emotional support, brief psychotherapy, and family-based interventions. Consult with Program Coordinators and referrers to develop psychiatric evaluations and treatment plans, ensuring seamless care coordination. Provide education, consultation, and support to VNS Health staff, empowering them to better care for patients with psychiatric, cognitive, and/or behavioral problems. Participate in interdisciplinary team meetings, case conferences, and clinical supervision to drive patient care and professional growth. Maintain accurate and up-to-date records in accordance with AMH and VNS Health record-keeping standards, ensuring compliance and high-quality care. Assume responsibility for continued professional growth, staying abreast of best practices and industry developments. Essential Qualifications To succeed in this role, you'll need: A current registration to practice as a Registered Nurse in New York State. A Bachelor's Degree in Nursing, with a preferred Master's Degree in Psychiatric Nursing. A minimum of one year of recent inpatient, outpatient, or home care psychiatric experience. Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals. Strong assessment and analytical skills, with the ability to develop and implement effective treatment plans. Ability to work in a fast-paced environment, navigating varying situations and priorities. Preferred Qualifications While not essential, the following qualifications will make you a more competitive candidate: Bilingual skills, which may be required depending on operational needs. A valid driver's license, which may be required depending on operational/regional needs. Previous experience working in home care or community-based settings.
