**Job Summary**
We're seeking an experienced remote social worker to join our team of dedicated professionals. As a medical case manager and community resource coordinator, you'll work collaboratively with registered nurses and other clinical staff to assess, plan, and advocate for the needs of our patients. **Responsibilities**
* Collaborate with registered nurses and other clinical staff to assess, plan, and advocate for the needs of patients
* Research and connect patients with available options and resources for financial, transportation, safety, and housing needs
* Empower patients to make informed decisions about their health and connect them with essential resources
* Work as a team with other clinical staff to assist patients with community resources for medical, financial, and behavioral health
* Utilize a collaborative process of assessment, planning, and advocacy to meet patients' benefit plan and/or health needs
* Conduct telephonic or electronic exchanges with patients to assess their needs and provide support
* Develop and implement personalized plans to address patients' needs and promote their overall well-being
**Qualifications**
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence
* 2+ years of experience in medical social work or case management
* Masters degree in social work or a related field
