**Responsibilities**

As an experienced remote social worker at careerzynith, your key responsibilities will include:

* Collaborating with registered nurse case managers to assess, plan, and advocate for members' benefit plans and health needs
* Researching and connecting members with available options and resources for financial, transportation, safety, and housing needs
* Empowering members to make positive lifestyle and medical choices through education and support
* Working in tandem with other clinical staff to connect members with community resources for medical, financial, and behavioral health services
* Utilizing telephonic and electronic exchange to communicate with members and stakeholders

**Requirements**

To succeed in this role, you will need:

* An active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in your state of residence
* At least 2 years of experience in medical social work or case management
* A Master's degree in Social Work (MSW) or a related field
