Experienced Remote Case Manager Registered Nurse - Care Coordination and Health Management

About the Role This 100% remote Case Manager RN position is an exciting opportunity to work independently and make a meaningful impact on the lives of our members. As a remote employee, you'll have the flexibility to work from anywhere in the state you're residing in, with normal working hours from Monday to Friday, 8:00 am to 4:30 pm in your time zone, and occasional late shift rotations as needed. Key Responsibilities Utilize data-driven methods to identify members who require individualized case management programs and referrals to alternative healthcare programs. Conduct comprehensive clinical assessments to evaluate member needs and develop flexible approaches based on benefit plans or external programs/services. Develop and implement case management plans, monitoring and evaluating their effectiveness, and involving the Medical Director as necessary. Advocate for patients to ensure they receive the full extent of their existing health care coverage. Promote quality, cost-effective outcomes, and suggest improvements to program/operational efficiency. Identify and escalate quality of care issues through established channels. Provide exceptional customer service, utilizing assessment techniques to determine member health literacy, technology capabilities, and readiness to change. Employ influencing/motivational interviewing skills to engage members and promote lifestyle/behavior changes. Offer coaching, information, education, and support to empower members to make informed medical and healthy lifestyle choices. Essential Qualifications To succeed in this role, you'll need: An active, current, and unrestricted RN license in your state of residence, with a willingness to obtain additional state licenses upon hire (paid for by the company). A minimum of 3+ years of acute care experience as an RN, with Med-Surge experience preferred. Availability to work Monday through Friday, 8:30 am to 5:00 pm in your time zone, with occasional late shift rotations as needed. Associate Degree required; BSN preferred. Preferred Qualifications To stand out as a candidate, you'll ideally have: A Compact RN License or Multi-state licensure. Certified Case Manager (CCM) certification. Telephonic case management and/or telephonic experience. Skills and Competencies As a Case Manager RN, you'll need to demonstrate: Strong decision-making skills and sound judgment.
