About the Role
We're seeking an experienced Mental Health Practitioner to join our Community Competency Restoration team. As a Community Competency Restoration Specialist, you'll work closely with clients, families, and community agencies to provide comprehensive mental health services that promote recovery, stability, and overall well-being. This is a remote role, offering the flexibility to work from anywhere, and we're looking for someone who is passionate about making a difference in the lives of others.

Duties and Responsibilities

Work with the larger treatment team to provide services for clients, supporting the development, writing, implementation, evaluation, and revision of overall treatment goals and plans in collaboration with the client and the treatment team.
Develop an individualized crisis plan coordinated with community emergency support services.
Meet with clients to explain services, assess client needs, establish and implement a care plan to address those needs, and arrange for resources necessary for community transition.
Coordinate services with other county or community agencies, including but not limited to Mental Health Case Management, Adult Protection, and community hospitals.
Assist in the development of comprehensive assessments, individualized treatment plans, substance abuse treatment, and other tools necessary to assist clients in the community.
Assist clients with all aspects of psychiatric rehabilitation and all aspects of obtaining and maintaining housing, including teaching independent living skills such as cooking, shopping, cleaning, personal hygiene, transportation, and budgeting.
Attend commitment and other court hearings as needed, complete required court reports if requested, and file reports with Vulnerable Adult, Child Protection, the Ombudsman's Office, and other mandated/required reports when applicable.
Actively and assertively engage and reach out to clients' family members/significant others, including landlords, employers, and other treatment team members after obtaining client permission.
Provide on-call crisis intervention as determined by the Program Director, which may include after-hours phone calls.
Complete all paperwork, reports, and charting contemporaneously and in an organized manner.


Requirements
To be successful in this role, you'll need to have a Bachelor's degree in Psychology or Social Work, plus 2,000 hours of work experience providing services to adults diagnosed with a serious mental illness. Alternatively, you may have no college degree and 4,000 hours of supervised work experience in the delivery of services to adults or children with mental illness.
