Position Overview
As a Medical Social Worker at arenaflex, you will play a vital role in our interdisciplinary care team, focusing on the emotional and social well-being of patients receiving home health services. You will evaluate, develop, and implement personalized care plans that address the psychological, social, and environmental factors affecting each patient's health journey. This role offers the flexibility of Per Diem work while allowing you to build meaningful relationships with patients and their families in the comfort of their homes.

Key Responsibilities
The Medical Social Worker is responsible for providing comprehensive social work services to patients within the home health setting. Working collaboratively with our interdisciplinary care team, you will ensure that each patient's emotional and social needs are addressed alongside their medical treatment. Your expertise will be crucial in helping patients navigate the complexities of recovery, connecting them with essential community resources, and supporting their families throughout the care process.

Patient Assessment and Care Planning

Conduct thorough assessments of each patient's social and emotional state as it relates to their illness, injury, and response to treatment
Evaluate the interplay between patients' medical and nursing needs within their home environment
Identify financial resources, support systems, and community resources available to patients and their families
Develop and periodically re-evaluate personalized plans of care that address emotional and social needs
Participate actively in the development and modification of the physician's Plan of Care


Patient and Family Education

Educate patients and their families on treating and coping with social and emotional responses connected to their health conditions
Provide guidance on navigating healthcare systems, understanding treatment plans, and managing the psychological impact of illness or injury
Offer ongoing assessment of patient and family needs, ensuring continuous support and responsiveness to changing circumstances
Serve as a trusted resource for patients and families, answering questions and addressing concerns with empathy and professionalism


Community Resource Coordination

Identify and connect patients with available community resources to resolve issues that may impede recovery
Assist with discharge planning needs, ensuring smooth transitions between care settings
Help patients access housing, shelter, legal services, financial assistance programs, and medication assistance
Coordinate funeral and memorial arrangements when necessary, providing compassionate support during difficult times
Maintain up-to-date knowledge of community resources and referral services


Interdisciplinary Collaboration

Participate as an active member of the interdisciplinary care team in care coordination activities
Communicate effectively with physicians, nurses, therapists, and other healthcare professionals
Provide consultation and act as a resource to team members in identifying and resolving patient needs
Participate in case conferences and team meetings to discuss patient progress and care strategies
Document and report changes in patient condition and response to treatment to the Clinical Manager


Supervision and Documentation

Supervise, instruct, and evaluate the performance of Social Work Assistants (BSW) to ensure compliance with company, governmental, and professional standards
Maintain accurate and timely documentation, including case conferences, patient/physician contacts, visit reports, and progress notes
Confer with other healthcare disciplines to provide optimal patient care
Ensure all required documentation is submitted per company and facility policies


Required Qualifications

Education: Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education (CSWE)
Licensure: Active Social Worker licensure in the state of Washington; if required by state law or regulation
Experience: Minimum of one year of experience as a social worker in a healthcare setting, with preference for home health and/or hospice experience
Certification: Current CPR certification
Transportation: Valid driver's license, auto insurance, and reliable transportation are required
Language: Must read, write, and speak fluent English

Compensation and Benefits
arenaflex is committed to providing competitive compensation that reflects the value and expertise our team members bring to patient care. The pay range for this Per Diem position is $52,500 - $72,225 per year, based on a 24-hour per week schedule. Compensation may vary based on geographic location, individual experience, education, and demonstrated competencies.
