As a Licensed Social Worker in this remote role, you'll work with a registered nurse case manager to assess, plan, and advocate for members with financial, transportation, safety, and housing needs. You'll research available options and resources to empower members to make positive lifestyle and medical choices. This role requires an LSW/MSW with an unrestricted active license, and you'll work with a team of clinical staff to assist members with community resources for medical, financial, and behavioral health.

• Active and unrestricted LSW - Licensed Social Worker, LMSW - Licensed Masters Social Worker, LCSW - Licensed Clinical Social Worker, or LISW - Licensed Independent Social Worker in state of residence
• 2+ years experience in medical social work or case management
• Masters Degree in Social Work
