**Job Summary**
We're seeking an experienced Licensed Social Worker to join our remote medical case management team. As a Licensed Social Worker, you'll partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with members across the entire lifecycle, utilizing telephonic and electronic communication to empower them to make positive lifestyle and medical choices.
**Responsibilities**
* Research available options and resources for members with financial, transportation, safety, and housing needs
* Collaborate with registered nurse case managers to assess, plan, and advocate for member needs
* Utilize a collaborative process to meet member benefit plan and/or health needs
* Empower members to make positive lifestyle and medical choices
* Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
* Conduct telephonic and electronic communication with members
* Maintain accurate and up-to-date records of member interactions and outcomes
**Requirements**
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence
* 2+ years of experience in medical social work or case management
* Masters degree in Social Work
**Preferred Qualifications**
* Experience working with diverse populations, including children, adults, and families
* Knowledge of community resources and services for medical, financial, and behavioral health
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Experience with electronic health records and other healthcare software

* Competitive salary range: $46,597.20 - $101,228.40
