* Provide training, information, and support to empower individuals to make ongoing autonomous clinical or potentially healthy lifestyle decisions
* Assist individuals effectively and knowledgeably to participate with their supplier in healthcare management
**Essential Qualifications**
* Bachelor's degree in a related field (e.g., conduct health, human services, psychology, social work, marriage and family therapy, counseling)
* 2+ years of experience in conduct health or social services
* 2+ years of experience in Microsoft Office applications (Word, Excel, Outlook)
**Preferred Qualifications**
