We’re looking for a Diabetes Practitioner who’s ready to make a real impact by supporting those with Type-2 Diabetes bring blood sugar readings down to a healthy range over the course of a 1y programme.

Part of Acacium Group, Xyla's health and wellbeing team are dedicated to making the UK’s local communities healthier. We help people to understand and take control of their own health through sustained behaviour change, transforming lives for the better through weight management, quit smoking services, diabetes services, cardiovascular disease prevention and health coaching & social prescribing.
As a Diabetes Practitioner you will be coaching service users on the Type 2 Diabetes Remission program, delivering 1-1 coaching via Digital Phone sessions. A key focus will be to encourage and help service users to make behavioural changes, set healthy lifestyle goals and complete the programme, assisting them to overcome potential barriers that they are facing.

šŸ’” About the Role
Work closely with service users throughout the programme delivering a first class service and knowledge

Conduct 1-1 coaching sessions to service users via the phone, focusing on the 3 stages of the programme, total diet replacement, food reintroduction, and then maintenance.

Maintain data monitoring records of all service users and ensure that this is handled with the greatest care

Support the service user engagement strategy to ensure adherence to the programme and minimise drop-outs

šŸŽÆ What You’ll Bring
Minimum of undergrad Degree in Nutrition or Health Related Science (Sports/Exercise Science/Health Psychology OR experience in personal training with a level 2/3 PT qualification

Phone based experience & technologically adept with Microsoft packages & systems.

Speaking Urdu or Punjabi is a bonus

šŸ“š Experience and Qualifications We’re Looking For
Relevant health and wellbeing qualification such as (but not limited to) an undergraduate degree, diploma, or equivalent in Nutrition, Dietetics, Health Psychology, Sport Science and/or a CIMSPA fitness qualification at level 3 or above
