You will be a critical part of our team, working directly with clients and collaborating with other professionals to create individualized care plans.Responsibilities:Conduct assessments to understand the needs of clients and develop personalized case management plansCoordinate with service providers and community resources to ensure clients have access to necessary servicesMonitor clients’ progress, adjusting plans as necessary to meet their needs and goalsProvide emotional support and advocacy for clients, ensuring their needs are met in a timely and respectful mannerMaintain detailed case notes and documentation in accordance with company policies and industry regulationsRequirementsQualifications:A Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (Master’s degree preferred)Proven experience in case management or a related fieldStrong communication, organizational, and interpersonal skillsAbility to work independently and as part of a teamCommitment to a client-centered, compassionate approach to service deliveryWe look forward to seeing how you can contribute to our mission and grow within our organization.
