Responsibilities:
• Conduct medication reviews and assessments to ensure optimal therapeutic outcomes.
• Provide clinical consultation to healthcare teams and nursing staff on medication-related issues.
• Develop and implement individualized pharmaceutical care plans for patients.
• Monitor patient medication regimens for safety and efficacy, adjusting as necessary.
• Educate patients and staff on medication management and best practices.
• Stay current on pharmacy regulations and industry trends to ensure compliance.
• Collaborate with interdisciplinary teams to improve overall patient health outcomes.
• Participate in quality assurance initiatives and pharmacy audits.
Requirements:
• PharmD degree from an accredited pharmacy school.
• Active pharmacist license in California.
• Minimum of 2 years of experience in a clinical pharmacy setting.
• Strong knowledge of pharmacotherapy and medication management.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively in a fast-paced environment.
• Detail-oriented with strong analytical and problem-solving abilities.
