Complex Case Manager RN - Maternity, NICU (Remote)

Provide high-quality and compassionate care to high-risk maternity and NICU patients.
Utilize clinical expertise to assess, plan, and implement individualized care plans for patients and their families.
Serve as the primary point of contact for patients and their families, providing support and guidance throughout their medical journey.
Collaborate with interdisciplinary teams to ensure continuity and coordination of care for patients.
Use critical thinking skills to identify potential barriers to care and develop solutions to overcome them.
Conduct thorough assessments and evaluations to identify patient needs and develop appropriate care plans.
Educate patients and their families on medical conditions, treatments, and available resources.
Monitor and track patient progress, making adjustments to care plans as needed.
Communicate effectively with patients, families, and healthcare team members to ensure the best possible outcomes.
Maintain accurate and timely documentation of patient care and progress.
Stay up-to-date on current trends and best practices in maternity and NICU care.
Adhere to all regulatory and compliance guidelines.
Serve as an advocate for patients and their families, ensuring their voices are heard and their needs are met.
Act as a resource and mentor for other healthcare team members.
Demonstrate a commitment to continuous learning and professional development.
