The Community Supports Coordinator manages the member journey post-admission, ensuring authorized hours are utilized appropriately and services remain uninterrupted while leading proactive outreach to members and caregivers.ResponsibilitiesMonitor and manage member authorized hours to ensure optimal utilization and prevent service disruptionsLead the reauthorization process, including documentation collection and coordination with funding sources and health plansServe as the primary point of contact for members, caregivers, and funding partners regarding utilization and service continuityProactively conduct outreach to address underutilization, overages, and pending authorizationsManage and resolve Salesforce cases and inbound inquiries in a timely and accurate mannerMaintain accurate documentation of member interactions, service status, and authorization updatesCollaborate cross-functionally to escalate issues, uphold compliance requirements, and support closed-loop referral processesSkills1–3 years of experience in customer service, care coordination, or service-oriented rolesAbility to manage high volumes of communication and case activity in a fast-paced environmentStrong communication and relationship-building skillsAttention to detail and documentation accuracyAnalytical ability to interpret utilization trends and identify service gapsTime management and prioritization in high-volume workflowsProblem-solving and sound judgmentProficiency in Microsoft Office and CRM systemsCustomer-focused mindset with strong follow-throughExperience working in healthcare, home care, or regulated service environmentsFamiliarity with ticketing or case management systems (Salesforce preferred)Bilingual SpanishCompany OverviewOne of the largest, most trusted in-home care companies in the nation.
