Through home visiting, this individual assists clients in overcoming barriers to health, social services, education, and employment by connecting them to identified Pathways. The incumbent empowers clients to navigate medical, behavioral health, and social services by removing barriers to these services.QualificationsMinimum Requirements:
One (1) or more years of experience in social services or care coordination, specifically serving older adults' behavior health clients or adults with complex chronic conditions
CA Driver's License
Preferred Requirements:
Community health worker training or certification
Bilingual/bicultural Spanish
