Community Health Manager

Position Summary:
The Community Health Manager oversees and leads a team responsible for implementing and supporting strategies related to State Rural Health Transformation grants. This role focuses on strengthening brain health and care by advancing community‑based engagement efforts across diverse audiences. The Community Health Manager will build and leverage state relationships to support and guide the execution of activities that promote scalable, community‑anchored approaches designed to expand access to brain health and dementia‑capable care and improve the care experience for individuals living with dementia and their families through the engagement of health and social‑service professionals. In partnership with state stakeholders, the Community Health Manager supports the coordination of infrastructure and resources aligned with each state’s identified priorities, which may include:
- Workforce Development
- Community Resource Coordination and Navigation
- Brain Health Education and Risk Reduction

Responsibilities

Lead the implementation of the Association’s Rural Health Care Transformation strategies (state specific) including Dementia‑Capable Workforce, Community Resource Coordination and Navigation and Brain Health Education and Risk This includes supporting reporting requirements, meeting grant requirements and goals of specific initiatives.
Build scalable partnerships with FQHCs, health systems, clinics, rural health providers, and community organizations access and drive outcomes related to grant
Lead and supervise the team responsible for implementing the Association’s Rural Health program in the states assigned including staff related to professional and workforce development, community resource coordination and brain health education and risk reduction initiatives.

Qualifications

Bachelor’s degree required in public health, social work, health administration, nursing, gerontology, or related field.
3-5 years of progressive experience in public health, community health, chronic disease management, aging services, health system outreach, or related fields.
5+ years of leadership experience managing teams, preferably large‑scale/state program settings.

Knowledge, Skills and Abilities

Demonstrated experience working with FQHCs, primary care practices, health systems, rural health providers, and community‑based organizations.
Experience leading workforce development initiatives, including training, capacity‑building, or program implementation.
Proven success in clinical‑to‑community integration, care‑coordination models, or navigation programs.
Strong track record of stakeholder engagement, coalition work, and cross‑sector collaboration.
Dementia Care, Rural Health, Health Systems & Public Health Expertise
Familiarity with interdisciplinary professional roles in rural populations, such as community health workers (CHW), social workers, nurses, physicians, etc.
Proven ability to lead large‑scale state program implementation within health or community‑based systems.
Demonstrated ability to build and sustain strategic partnerships with state and other groups involving populations such as health systems, FQHCs, rural health organizations, public health agencies, and other community partners.
Ability to communicate effectively with leaders across clinical, care, public health, and community sectors.
Experience supervising, coaching, and developing a high‑performing team.
Ability to manage budgets, timelines, contracts, and complex state initiatives.
Strong project management skills with the ability to prioritize, delegate, and coordinate multiple workstreams.
Proficiency in using data systems, dashboards, and reporting tools to track program performance and outcomes.
Strong written and verbal communication skills, including the ability to translate complex concepts for diverse audiences.
Ability to travel up to 60%

Position Grade & Compensation: Grade 108 The Alzheimer's Association’s good faith expectation for the salary range for this role is between $64,000 - $80,000
