Learn more about us here.We connect community members with essential services and supports that improve quality of life, reduce barriers, and strengthen resilience.RequirementsPosition Overview:The CHI Specialist supports patients with identified social determinants of health (SDOH) needs through outreach, resource navigation, and care coordination under the general supervision of the billing practitioner. Work is non-clinical unless appropriately licensed. Key Responsibilities:Patient Outreach & EngagementConduct telephonic or digital outreach to assigned patientsPerform standardized SDOH screeningsIdentify barriers such as food, housing, transportation, and utilitiesEngage patients using approved engagement techniquesResource NavigationConnect patients to appropriate community-based resourcesFacilitate referrals and warm handoffsAssist with applications for social services when appropriateTrack closed-loop referral completionCare Plan SupportReinforce practitioner-directed care plansDocument patient goals and progressEscalate clinical or safety concerns to RN or practitionerMaintain accurate monthly time logsDocumentation & ComplianceRecord all encounters in the designated platform/EHRMaintain HIPAA compliance at all timesFollow incident-to and CHI program requirementsParticipate in QA reviews and required orientation updatesQualifications:Minimum:High school diploma or equivalentStrong phone-based communication skillsAbility to work independently in a remote environmentPhysically located in the United StatesHIPAA-compliant home workspaceRequired prior to independent case work:Must successfully complete CircleLink CHI orientation and competency validation before independently performing billable CHI servicesMust adhere to all company protocols, scripts, and documentation standardsPreferred Qualifications (Not Required)Experience in care coordination, community health, social services, or patient navigationCommunity Health Worker (CHW) certificationMedical Assistant, LPN, RN, or Social Work backgroundExperience with Medicare care management programsBilingual (Spanish strongly preferred)Experience working with high-risk or Medicare populationsTechnical Requirements (Remote Work)Private, secure home workspaceReliable high-speed internetComputer capable of running required systemsHeadset for patient callsAbility to access systems via secure login/VPNCompliance with all HIPAA safeguardsSupervision & ScopeWork performed under general supervision of the billing practitionerMust escalate any clinical or safety concerns immediatelyMay not provide clinical advice unless appropriately licensed and authorizedMust complete required orientation and maintain competency status Productivity ExpectationsMeet monthly case completion targetsMaintain quality and documentation standardsAchieve required patient engagement metricsParticipate in periodic orientation updates and QA review Compliance Statement This role supports Medicare Community Health Integration services and must adhere to incident-to requirements, HIPAA regulations, applicable state scope-of-practice laws, and CircleLink policies.BenefitsWhat We Offer:$21.00 per completed 60-minute CHI casePaid per qualifying monthly case meeting documentation and time standardsAdditional time units (if applicable) compensated per company scheduleNo payment for incomplete or non-billable encountersFraud Alert:CircleLink Health has identified fraudulent emails sent from @joincirclelinkhealth.team, which is NOT affiliated with our company.
