Collaborative Care Management LCSW remote FT/PT

Job Overview We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team. The ideal candidate will possess a strong background in mental health counseling and be adept at providing comprehensive support to our staff caring for individuals in the Collaborative Care Program. This role involves oversight, guidance and education with our remote COCM Care Managers and our patients . Our goal includes ensuring that our patients receive the highest standard of care through various therapeutic modalities. Responsibilities • Educate and monitor the staff to conduct thorough patient assessments and diagnostic evaluations to determine appropriate treatment plans. • Provide individual and group counseling sessions with patients and educational inservice with our staff to teach how to utilize techniques such as cognitive behavioral therapy, psychodynamic therapy, and motivational interviewing. • Develop and implement treatment plans tailored to the unique needs of patients including those experiencing PTSD, addiction issues, or developmental disabilities. • Facilitate crisis intervention and management strategies for patients in acute distress. • Maintain accurate medical documentation, progress notes, and treatment updates in compliance with state and federal regulations. • Collaborate with interdisciplinary teams to ensure comprehensive patient care and effective case management. • Engage in clinical research activities to enhance service delivery and contribute to the field of social work. • Utilize EMR systems for documentation and data collection related to patient care outcomes. Experience Candidates should have a robust background that includes: • Experience in psychotherapy, particularly with the mental health population • Familiarity with behavioral therapy techniques and applied behavior analysis. • Knowledge of medical terminology, CPT coding, ICD-9/ICD-10 coding practices. • Previous hospital experience or work in geriatrics is preferred. • Strong understanding of clinical research methodologies related to social work practices. • Proven ability in crisis management and intervention strategies.
