Your role will focus on personal care, mobility assistance, and health monitoring to enhance the well-being of clients in their homes. Essential Duties Provide personal care (bathing, dressing, grooming, toileting, mobility support) Assist with safe transfers, repositioning, and ambulation Support activities of daily living (ADLs) in a dignified and professional manner Prepare meals and assist with feeding as needed Perform light housekeeping (laundry, dishes, maintaining a clean and organized space) Provide transportation for medical appointments, errands, and social outings Monitor and report health changes to the Care Coordinator Maintain accurate documentation in electronic medical records Apply Today & Start Making a Difference! Requirements Preferred Requirements Current CNA Certification (Required) Previous experience in home care, hospice, or skilled nursing preferred A passion for helping others and providing quality care Reliable transportation & a valid driver’s license (Required for home visits) Ability to travel within Cape Cod Ability to pass background checks, work-related references, and drug testing Strong communication and interpersonal skills Work Environment & Physical Demands Client Home Setting – Work in private homes, traveling between client locations as needed Physical Activity – Regular standing, walking, and assisting with transfers (Must be able to lift up to 25 pounds)
