Direct Care and Treatment (DCT) is seeking an individual to join our Forensics Clinical Team for an exciting and rewarding opportunity as a Social Worker ! This position will provide consultation and care coordination for complex discharges from the Forensic Mental Health Program through direct collaboration with patients, multidisciplinary team members, financial services, and community partners to promote discharge plans that meet patient needs, manage risk factors, and support recovery. Responsibilities include: + Providing psychosocial services to patients, groups, and families, and assisting and participating in the planning and revision of the ongoing treatment process. + Communicate information needed to facilitate cooperative treatment planning with families, counties, and external agencies. + Collaborate with county human services in securing appropriate community-based resources on behalf of the patient. The Forensic Services campus in St. Peter is comprised of the Forensic Mental Health Program and the Forensic Nursing Home. Forensic Services provides mental health evaluation and treatment services to individuals diagnosed with a mental illness who present a risk to public safety. This is a safe and secure treatment environment. **About the Saint Peter Community:** **Established in the Minnesota River Valley, St. Peter is a family-oriented, growing community of around 10,000 people. It is located in South Central Minnesota, approximately 60 miles southwest of the Twin Cities and 12 miles north of Mankato. St. Peter was founded in 1853, and it is one of the oldest cities in the state. Over 40 sites in town are listed in the National Register of Historic Places. St. Peter has a quaint and well-maintained downtown area filled with small businesses, such as excellent bakeries, coffee shops, and restaurants. Moreover, Mankato hosts an orchestra, Carnegie Art Center, and several live music venues.** **Minimum Qualifications** + Bachelor's degree in social work; AND + Licensure by the State of Minnesota to practice as a Licensed Social Worker (LSW); AND + Two (2) years of post-bachelor's degree experience providing behavioral health clinical services including at least 3 of the 5 activities listed below:¿ ¿ OR + Master's degree in social work; AND + Licensure by the State of Minnesota to practice as a Licensed Graduate Social Worker (LGSW) AND + One year of post-master's degree experience providing behavioral health clinical services, including at least 3 of the 5 activities listed below:¿ ¿ OR + Master's degree in psychology, social work, behavior analysis, or related field;¿ + ¿Licensure/Certification in the State of Minnesota (LICSW) **Preferred Qualifications** + Experience collaborating with Minnesota counties (AMHI.CMH), community hospitals, and state psychiatric/forensic facilities. + Working knowledge of community mental health resources and the continuum of care for individuals with serious and persistent mental illness (SPMI). + Knowledge of culturally appropriate community-based services and resources for diverse population. + Understanding of funding mechanisms that support people with disabilities (e.g., Medical Assistance, Housing Support/GRH, Medicare). + Proficiency in identifying and securing home and community-based services (HCBS) waivers, specifically CADI, BU, and Elderly Waivers. + Familiarity with Housing Stabilization Services and experience working with specialized providers for Community Residential Services (CRS). + Our employees are dedicated to ensuring cultural responsiveness.
