Job Description Job Description Weber Human Services (WHS), located in Ogden, Utah serves a growing urban population residing in Weber and Morgan Counties. We are an organization dedicated to helping those in need rebuild their lives and reach their full potential. WHS was organized by a cooperative agreement between the Weber and Morgan county governments to serve as the local authority for the provision of their substance abuse, mental health, and senior service programs. Weber Human Services is the only organization in the State of Utah providing these services under a single administration. Through dozens of human services programs, WHS helps thousands of its neighbors by supporting and empowering our most vulnerable groups, including at-risk youth, the elderly, children, youth and adults suffering from mental illnesses, and those recovering from addictions. As a Certified Case Manager for our Adult Mental Health Team, you will work with adults who need community resources. You will need to have experience working with mental illness and be willing to work in the community, schools, hospitals, residences, shelters, etc. You must have strong collaboration and communication skills and be willing to work a flexible schedule if needed. We will provide training to become a Certified Case Manager. Spanish speaking is a plus, and a stipend will be available upon passing the Spanish competency test. You will be driving and need a valid Utah Driver's License. TCM Services to Outpatient Clients: Knowledge of the Medicaid Provider Manual. Knowledge of agencies, services, and programs within the community. Ability to teach daily living skills from evidence-based material. Transportation to provide direct service, Documentation, Coordination, Trainings and Meetings: Excellent chart management skills. Ability to document services within agency standards. Attendance at monthly adult team meetings, monthly supervision, weekly TCM meetings, and quarterly general staff meetings. Attendance at training as required. Social Service Worker License preferred. Certified Case Manager within 1 month of hire date and renew every 3 years. Valid Utah Driver's License. Defensive Driving and clearance to operate WHS vehicles.
