Are you passionate about making a lasting impact in the lives of families facing serious challenges? Join our dedicated team as a Case Planner in the Prevention and Special Medical Program, where you'll work directly with families at risk of foster care placement due to substance use or mental health disorders. Responsibilities: Guide families through a structured, three-phase treatment model: Initial, Baseline, and Stabilization. Conduct comprehensive assessments and develop individualized service plans. Coordinate multidisciplinary interventions tailored to each family's unique needs. Build meaningful relationships with families over an average case duration of 12 months. Collaborate with internal and external providers to ensure safety, stability, and well-being. Requirements: BSW or bachelor's degree in human services. Some knowledge of the child welfare system.
