Case Manager - Scott County

Pinnacle Services, Inc. is an innovative agency experiencing rapid growth in the Case Management department. They are seeking friendly, self-starting professionals to join their Hennepin County Case Management team, where Case Managers will work with individuals facing various challenges and are responsible for assessing needs, offering resources, and monitoring service delivery. Responsibilities Driving to attend client meetings in Scott County, with an average of 4 client meetings per week Assessing the areas of the individual's life where assistance is needed Offering and coordinating resources for the individuals being supported Monitoring service delivery and working with providers to ensure the individual's needs are met Service plan development Attend regular in-person meetings with individuals served and their interdisciplinary teams Case Noting client-related work to create Billable Units Skills Be a graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field Be a graduate from an accredited four-year college with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency Applicants must have a valid driver's license and acceptable driving record as determined by Pinnacle Services policy Applicants must have a vehicle that is covered with current insurance Applicants must successfully clear local and/or stated background checks Applicants must be willing to commute to areas of Scott County for potential meetings One year of experience in education or treatment of people with developmental disabilities or related conditions, or a minimum of one course that specifically focuses on developmental disabilities Possess a LSW or higher form of BOSW Licensure Benefits Training Program - up to 8 weeks Health Insurance Dental Insurance Vision Insurance Life Insurance Short Term Disability Voluntary Life Insurance MN Paid Leave Flexible Spending Account Paid Time Off 7 Holidays 401K Eligible to contribute 6 months from start date.
