Conduct comprehensive assessments of members' health and healthcare needs Develop individualized care plans based on members' needs, goals, and preferences Collaborate with healthcare providers and other members of the care team to ensure coordinated and effective care Monitor and evaluate members' progress towards achieving their health goals Identify and address any barriers to care, such as social determinants of health or access to resources Educate members on their health conditions, treatment options, and self-management strategies Serve as an advocate for members, ensuring their voices are heard and their needs are met Document all care management activities and maintain accurate and up-to-date records Adhere to all regulatory and compliance requirements related to case management Continuously seek opportunities for improvement and implement best practices to enhance the quality of care provided to members Collaborate with other healthcare professionals and community resources to promote wellness and prevent hospital readmissions Communicate effectively with members, their families, and healthcare providers to facilitate seamless care transitions and improve health outcomes.
