Case Manager, Registered Nurse
Description:
• Coordinate care for Special Needs Plan (SNP) members with chronic medical and behavioral health conditions and SDoH needs
• Conduct comprehensive assessments and develop individualized care plans
• Provide education and disease management support
• Collaborate with interdisciplinary care team and ensure accurate documentation
• Identify and connect members with health plan benefits and community resources
• Meet regulatory requirements and participate in team meetings
• Support team through clinical decision-making and participate in projects and process improvements
Requirements:
• Active and unrestricted Registered Nurse (RN) licensure in the state of Missouri OR compact licensure in state of residence
• Proficient in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams
• Access to a private, dedicated space to conduct work effectively
• Confidence working at home / independent thinker
• 3+ years of nursing experience
• 2+ years of case management, discharge planning and/or home healthcare coordination experience
• Familiarity with community resources and services
• Excellent customer service and engagement skills
• Ability to meet performance and productivity metrics
• Experience or willingness to learn care management within Medicare and Medicaid managed care
