Case Manager - Hennepin/Scott County
Pinnacle Services, Inc. is an innovative agency experiencing rapid growth in the Case Management department. They are seeking friendly, self-starting professionals to join their Hennepin/Scott County Case Management team, focusing on providing high-quality services to individuals with various needs. Responsibilities Assessing the areas of the individual's life where assistance is needed Offering and coordinating resources for the individuals being supported Monitoring service delivery and working with providers to ensure the individual's needs are met Service plan development Attending regular in person meetings with individuals served and their interdisciplinary teams Case noting individual served related work to create billable units Attending two-thirds of client meetings in Hennepin County Attending one-third of client meetings in Scott County Skills Be a graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field Be a graduate from an accredited four-year college with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency Applicants must have a valid driver's license and acceptable driving record as determined by Pinnacle Services policy Applicants must have a vehicle that is covered with current insurance Applicants must successfully clear local and/or stated background checks Possess a LSW or higher form of BOSW Licensure Benefits Training Program - up to 8 weeks, faster paced training with learning curve Health Insurance Dental Insurance Vision Insurance Life Insurance Short Term Disability Voluntary Life Insurance Flexible Spending Account MN Paid Leave Paid Time Off 7 Holidays 401K - Eligible to contribute the 1st quarter after 6 months of employment Animal Friendly Corporate Office Professional Growth Opportunities Team Atmosphere MSSA membership & paid CEUs Licensing supervision Flex-time available after the first 60 days.
