Overview: We are seeking a motivated and organized Case Manager to join our skilled nursing facility team in a remote capacity. As a Case Manager, you will play a crucial role in ensuring the continuity of care by managing continued stay requests and insurance updates with insurance companies. This position requires excellent organizational skills, efficiency, strong computer literacy, and exceptional time management abilities.

Responsibilities:
• Manage and submit continued stay requests to insurance companies in a timely manner.
• Update insurance companies on patient progress and treatment plans as required.
• Review and document insurance coverage and reimbursement details accurately.
• Collaborate with healthcare providers to gather necessary clinical information for insurance submissions.
• Monitor and track insurance authorization timelines and requirements.
• Maintain comprehensive and organized records of insurance communications and patient information.
• Utilize electronic health record (EHR) systems proficiently to input and retrieve patient data.
• Ensure compliance with regulatory requirements and standards related to insurance authorization processes.
• Communicate effectively with internal team members and external stakeholders to facilitate smooth care transitions.
Requirements:
• Proven experience as a Case Manager in a healthcare setting, within a skilled nursing facility or similar environment.
• Experience in doing SNF authorizations and continued stay requests
• Experience in doing Part B therapy authorizations
• Strong understanding of insurance authorization processes and requirements. Understanding of Medicare, Medicaid, Insurance payer sources.
• Proficiency in using computer systems and EHR software; comfortable with technology and adapting to new platforms.
• Ability to work independently and remotely while maintaining high productivity and attention to detail.
• Effective communication skills, both written and verbal.
• Commitment to maintaining patient confidentiality and adhering to ethical standards in healthcare.
