Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Utilize care management tools and resources to support members' needs and goals
Requirements:
Minimum 2 years of experience in behavioral health or social services
Willingness to travel up to 50-75% of the time to meet with members in person
Reliable transportation and ability to reimburse mileage according to company policy
Active IL BH clinical license (LCSW, LCPC, LMFT)
Preferred Qualifications:
Case management and discharge planning experience
Managed care experience
Microsoft Office experience
