Case Manager (1099 Contractor, Remote – South Carolina)

About Us: Pathways To Promise Foundation is a 501(c)(3) nonprofit dedicated to transforming foster care by keeping siblings together and supporting families through Medicaid-based community services. We work closely with DSS, DJJ, and Medicaid programs to provide high-quality case management and support. Role Overview: We are seeking Contract Case Managers (1099) to join our growing team. In this role, you’ll work directly with youth and families referred through DSS, DJJ, and Medicaid, providing community support services (CSS). This is a flexible, remote position with required community-based client interaction in South Carolina. Key Responsibilities: • Provide case management and community support services per Medicaid guidelines (16 units per client, per week). • Contractor will maintain an active caseload of six (6) clients minimum • Person-Centered Planning (PCP) development and implementation. • Coordination of services for youth and families involved with DSS, DJJ, and related systems. • Participation in family team meetings and service planning sessions. • Crisis prevention and intervention planning. • Assistance with connecting clients to housing, education, employment, healthcare, and other community resources. • Documentation and timely submission of service notes in compliance with Medicaid/MCO standards. • Coordination and authorization with Managed Care Organizations (MCOs). • Attend virtual supervision, team meetings, and required trainings. ( When required) Qualifications: • Bachelor’s degree in social work, Human Services, Psychology, Counseling, or related field (required). • Minimum 2 years of experience in social services, case management, or community support services. • Experience using Microsoft Word, Excel, and Teams. • Knowledge of Medicaid documentation standards, authorizations, and service delivery requirements. • Familiarity with Person-Centered Planning and crisis prevention planning. • Ability to manage documentation, scheduling, and communication remotely. • Must pass background check. • Must provide personal laptop, phone, and have sufficient Wi-Fi for remote work. Contract Details: • 1099 Contractor role (no benefits). • $19.00/hour. • Flexible schedule (Fully Remote) • Work begins after Medicaid provider approval and client assignment.
