CASE MANAGEMENT NURSE
Job Responsibilities
The Case Management Nurse at Prime Dx is pivotal in managing patient care and ensuring optimal outcomes through effective coordination of health services. This role involves assessing, planning, implementing, monitoring, and evaluating options and services to meet an individual's health needs through communication and available resources.
Key responsibilities may include:
Assess new and ongoing cases, determining appropriate care and management plans.
Coordinate with healthcare providers, patients, and families to implement care plans effectively.
Monitor patient progress and adjust care plans as necessary.
Facilitate communication between patients, family members, and healthcare professionals.
Advocate for patient needs and ensure that they receive appropriate and timely medical care.
Document and maintain accurate records of patient care, condition, progress, and concerns.
Performs other duties as assigned.
