In this fully remote role, you'll leverage your skills to provide comprehensive social and medical case management support, empowering individuals to navigate healthcare and access vital resources. Key Responsibilities:

Provide comprehensive social and medical case management support to members across the U.S.
Manage and prioritize a caseload of member cases, ensuring timely and effective interventions.
Conduct thorough assessments to determine member eligibility for case management services.
Connect members with appropriate resources, including specialty programs, educational materials, and community support services.
Conduct research and provide guidance on member issues, including referrals to legal, financial, and community support services.
Collaborate with colleagues and clinical staff to achieve optimal member outcomes.
Maintain accurate and complete documentation of all member interactions and case details.
Participate in quality improvement initiatives to enhance member care.


Qualifications:

Proven experience in social and medical care.
Strong customer service and communication skills.
Proficiency in using computers and navigating various systems.
Bachelor's degree or equivalent experience.

Preferred Qualifications: Prior experience in managed care, psychology, or social work; Bilingual abilities.
