GHR has a wonderful Care Management RN opportunity for a Registered Nurse working remote in the PA TRI STATE AREA!
MUST have a PA, NJ or DE license
Registered Nurse Job Details:
Pay Rate: Starting at $40/hour
Shifts Available: Full Time 9a-5p
Infusion Therapy Team
Perform telephonic or onsite review of hospital admissions
Encourage efficient and high quality use of the healthcare delivery system
Promote timely and dynamic discharge planning to facilitate early discharge
Efficiently collaborate with the hospital utilization review department
Registered Nurse Job Requirements:

Minimum 3 years of acute care clinical experience in a hospital or healthcare setting
Prior discharge planning and/or care management experience REQUIRED
