Utilize strong organizational skills to maintain accurate and up-to-date records and documentation for each member. Monitor and track member progress and effectively communicate updates to all relevant parties. Collaborate with healthcare providers and insurance companies to advocate for members and ensure they are receiving appropriate care. Conduct in-depth assessments of members' health and well-being to identify any potential risks or issues that require attention. Develop and implement care plans for members, taking into consideration their individual needs, preferences, and resources. Educate and support members in managing their health and navigating the healthcare system. Serve as a liaison between members, healthcare providers, and insurance companies to facilitate efficient and effective communication.
