Coordinate and facilitate care for Aetna members, ensuring they receive timely and appropriate healthcare services.

Collaborate with healthcare providers, insurance specialists, and other team members to develop and implement personalized care plans for members.

Conduct thorough assessments of members' healthcare needs and identify any potential barriers to receiving quality care.

Communicate and build relationships with members to ensure their understanding and compliance with care plans and provide support and guidance as needed.

Monitor and track members' progress and outcomes, making necessary adjustments to care plans as needed.

Utilize Aetna's resources and tools to assist members in navigating their healthcare options and accessing necessary services.

Adhere to Aetna's policies and procedures, as well as industry regulations and standards, to ensure compliance and quality of care.

Maintain accurate and detailed records of members' care plans, assessments, and interactions for reporting and documentation purposes.

Collaborate with other team members to identify opportunities for improving members' health outcomes and overall satisfaction with their healthcare experience.

Continuously strive to improve and enhance Aetna's care management processes and services to meet the evolving needs of our members.
