Your Role
• Provide monthly calls and support to patients enrolled in the CCM program.
• Utilize your nursing skills to educate patients and their families.
• Monitor patients progress and work with the physicians to ensure quality patient care.
Qualifications
• Active California LPN license - MUST be a California license to qualify***
• You must live in a state within MST or PST to qualify
• Experience in geriatrics or primary care
• Ability to sit and talk on the phone for long periods of time
• System: We require you to have an i5 or newer Windows processor and to have Windows Pro (not Home). This can be a laptop or desktop.
• Must live/work full-time in the US - you can work from anywhere within the US but not outside.
The Pay
• The LPN pay ranges from $24+ per hour based on experience. (40hrs/week)
• The pay is a base pay plus a bonus per call. On average our Care Coordinators complete between 16 and 21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter.
